QS证书变更、注销、遗失、补副本等工作注意事项
The QS certificate and logout, lost, fill a copy of work notice
一、企业地址变更：
One, company address change:
　　（一）厂房搬迁新址的，属重大项变更；
( a ) plant relocated, is a major change;
　　（二）厂房在原地，但原址改名，不属重大项变更。

　　1、变更表3份
In 1, alter table 3
　　2、营业执照、卫生许可证、代码证复印件3份（变更后的）

　　3、工商证明复印3份（证明是同一个企业）

　　4、QS证正本、付本复印件3份

　　5、属厂房搬新址的，要组织现场审查，所有材料附后报省局（与发证前的现场审查一样）

　　二、企业名称变更：不属于重大项变更

　　1、变更表3份

　　3、工商证明复印3份（证明是同一个企业）

　　三、扩项：
Three, the expansion of:
　　1、填申请书（只填新扩的项目），然后按原来申证的一套程序完完全全地走一边，扩项暂时不收费

　　四、获证企业增加场地（在另一地址，但必须是在同一区，不跨地区，如获证企业在该地址另领执照，则视为另一独立法人，该新法人需重新申证。）

　　五、证书遗失

　　企业因毁坏或者不可抗力等原因造成生产许可证遗失或者无法辨认的，企业应提交补证申请及在省级以上报纸刊登的声明。

　　六、补副本：

　　1、企业写证明材料，说明情况，法人代表签字并盖企业公章
